The treatment of bronchial carcinoma by lobectomy and sleeve resection of the main bronchus.

چکیده
In 1947 Price Thomas performed a bronchotomy and removed a portion of the circumference of the wall of the right main bronchus for an adenoma, performing a plastic repair of the bronchus (Price Thomas, 1956). The patient had a straightforward convalescence, with good primary union of the bronchus, and has continued to make good use of both lungs ever since. The procedure has been repeated with success on several occasions. In 1952 he carried the idea further, in the treatment of a patient with a stricture of the main bronchus, in association with a destroyed tuberculous left upper lobe. In this case, the lower lobe was saved by resecting the stricture and anastomosing the divided ends of the main bronchus. The patient had a positive sputum at the time of operation, and subsequently developed tuberculous endobronchitis at the suture line, spreading distally, so that the pneumonectomy had to be completed. Nevertheless, it had been established that when two ends of a bronchus were

Lobectomy with sleeve resection in the treatment of bronchial tumours.
Bronchial carcinoma, situated at the level of the upper lobe orifices, may be treated surgically by pneumonectomy or lobectomy combined with sleeve reseotion of the main bronchus and end-toend anastomosis of the proximal and distal stumps, thus conserving the remainder of the lung. Many authors have emphasized the safety and wider applicability of sleeve resection. This procedure has a particul...
متن کاملDeeper wedge resection and parenchymal-sparing bronchoplasty of the secondary carina: an alternative surgical technique for removal of tumor located at the orifice of upper lobar bronchus
BACKGROUND Sleeve resection and reconstruction of the bronchial corner between the upper lobar bronchus and the intermediate bronchus is technique demanding. CASE PRESENTATION A 33-year-old Chinese man suffered from recurrence of low-grade malignant mucoepidermoid carcinoma located at the orifice of upper lobar bronchus with invasion to the main bronchus nearly 1 year after he had undergone a...
متن کاملUse of lung-preserving surgery in left inflammatory bronchial occlusion and distal atelectasis: preliminary results.
OBJECTIVES Lung-preserving surgery was proved to be effective and safe to treat patients with benign bronchial strictures. However, this surgical treatment has been rarely reported in patients with complete occlusion in the left main bronchus. The aim of this study was to assess the value of this procedure and report our experience in the treatment of these patients with left atelectasis caused...
متن کاملSleeve resection for bronchial carcinoid tumour in two children under six years old
BACKGROUND Paediatric tracheobronchial tumours are very rare, and pneumonectomy and lobectomy procedures are rarely indicated due to their surgical difficulties and high sequelae. Bronchoplastic techniques preserving lung parenchyma allow the resection and reconstruction of the main bronchi and carina. CASE PRESENTATION Here, we present a 6-year-old boy suffering from a carcinoid tumour of th...
متن کاملBronchial sleeve resection for early-stage squamous cell carcinoma
A 75-year-old man complained of sputum and was referred to our department. His sputum cytology was class III. Chest X-ray and computed tomography showed no abnormalities, but bronchoscopy revealed an elevated lesion in the membranous portion of the left main bronchus, which was pathologically diagnosed as squamous cell carcinoma in situ. Since bronchoscopy revealed no other lesions in the visib...
